چکیده

The prevalence of cholesterol gall stones in young women has increased since the introduction of oral contraceptives. The synthetic female sex hormones used in these preparations, increase the degree of cholesterol saturation in bile. To determine whether oestrogens, progestagens, or both, are responsible for the change in biliary cholesterol saturation index, a prospective randomised, controlled study was performed. A significant increase in the cholesterol saturation index of bile was observed when either 30 ug ethinyloestradiol plus 150 ,g norgestrel (p=001) or 50 ,ug ethinyloestradiol plus 250 gg norgestrel (p<0.01) were ingested daily for two months. No change in the cholesterol saturation index was observed when 30 ,g ethinyloestradiol alone, or 30 ,ug ethinyloestradiol plus 2.5 mg norethisterone were used. The mechanism for the increase in cholesterol saturation index did not appear to involve bile acid metabolism. These results indicate that the progestagen, norgestrel, and not as previously thought the oestrogen, ethinyloestradiol, is responsible for the increase in cholesterol saturation of bile which accompanies the use of oral contraceptives.

The effect of oral contraceptive steroids on bile secretion and bilirubin Tm in rats.

1. The effect of oestrogens and progestogens and their 17alpha-ethinyl derivatives on bile flow, maximum rate of bilirubin secretion, serum and liver bilirubin has been studied.2. Both 17alpha-ethinyl substituted oestrogens and progestogens greatly reduced the basal bile flow. The parent compounds, oestradiol-17beta and 19-nortestosterone had little or no effect.3. A much larger dose of progest...

Fluctuations of serum and bile lipid concentrations during the menstrual cycle.

We measured fasting serum and bile lipid concentrations at three intervals during the normal menstrual cycles of 11 healthy women not taking oral contraceptives. In nine of them cholesterol saturation of bile, and therefore presumably the risk of developing gall stones, was higher nine days after midcycle than at the end of menstruation. This change in bile cholesterol saturation was preceded b...

Gynaecological uses of dienogest alone and in combination with oestrogens

Dienogest is a hybrid synthetic gestagen which shares characteristics with 19-nortestosterone and progesterone derivatives, and also has advantageous substance specific pharmacological features. These include a high oral bioavailability, strong suppressive effect on the endometrium, lack of oestrogenic and androgenic effects, with a significant antiandrogenic efficacy component. Due to its reli...

Occasional reviews Do exogenous oestrogens and progesterone influence asthma?

Between 20% and 40% of women of childbearing age in the UK use hormonal contraceptives. 2 In a survey carried out in Oxfordshire in 1990 about 20% of women aged 45–64 years were taking hormone replacement therapy (HRT). Around 5% of women of childbearing age in the UK have diagnosed asthma, and a higher proportion have asthmalike symptoms. There is less information available about the prevalenc...
